How to Nominate

If you would like to be considered for a consumer representative position advertised in HealthUpdate or on CHF’s website then:

1. Consider whether or not you meet the basic requirements for CHF nomination – are you a member of a health consumer group, preferably a voting member organisation of CHF? Would this organisation support your nomination?
   

2. If so, then complete the CHF Nomination Form. If you are a member of CHF or an existing CHF consumer representative, then please go to the Members’ Area, where you can download the nomination form or complete it online. If you do not have access to the Members’ Area, then please contact CHF to request a copy of the form.
   

3. Return the form to CHF by the deadline shown. Your form should be accompanied by a current CV and a letter or an email of endorsement from your consumer group.
   

4. Your nomination will be forwarded to CHF’s Consumer Representatives Sub-Committee who will select the successful representative from the nominations received. CHF will inform you of the outcome. If you are not successful this time, then keep trying!
